Systemic Gonococcal Infection

As mentioned above, Gonorrhea is one of the common Sexually Transmitted Infections (STI) that is spreading at a very high rate in the sexually active group.

Gonorrhea is a sexually transmitted bacterial infection that is caused by Neisseria gonorrhoeae. It can infect both males and females. The Neisseria gonorrhoeae mostly targets urethra, throat or rectum. While in some cases, it is seen that the infection spreads to the bloodstream and various parts of the body leading to a condition known as systemic gonococcal infection.

  • Systemic gonococcal infection is also known as “disseminated gonococcal infection (DGI)”.

Incidence of Disseminated gonococcal infection (DGI) is very rare in the present era. Despite the decreased chances of its occurrence, it still remains one of the very important topics of discussion as it contributes to the high morbidity and mortality conditions in the sexually active generation. It presents various symptoms according to the part of the body that is infected.

The cause of systemic gonococcal infection is spread of the gonorrhea infection into the bloodstream. The bacterium that is responsible for systemic gonococcal infection is a gram negative bacterium known as Neisseria gonorrhoeae”.

The mode of spread is majorly through sexual contact with an infected person and it may get transferred from infected pregnant mother to the child during birth.

The infection may further spread to the bloodstream resulting in presentation of different symptoms.

  • Gonorrhea presents various symptoms like discharge through penis, burning and pain during urination however the symptoms may differ from person to person. Usually Systemic gonococcal infection produces mild systemic illness with fever.
  • Two main features of the Systemic gonococcal infection are a skin rash and tenosynovitis i.e. inflammation and swelling of a tendon, typically in the wrist.
  • Rash can be observed below the neck, which also involves the areas of limbs, palms and soles. Usually the rashes are not present on the face, scalp and mouth. The rashes result in skin lesions which usually resolve within 4 to 5 days without leaving behind a scar.
  • Pain and swelling in joints is experienced. Most commonly involved joint is the knee joint and the hip is rarely involved.

The infection is diagnosed by clinical examination as well as laboratory diagnosis.

Clinical examination is done by your doctor by observing your symptoms while the laboratory examinations include monitoring the state and levels of various components of the body such as-

  1. ESR levels
  2. WBC count
  3. Fluorescent antibody test
  4. Synovial fluid culture
  5. Skin culture etc.

Systemic gonococcal infection being a bacterial sexually transmitted infection, a variety of antibiotic are considered in the treatment regime of this condition. Penicillin is considered the drug of choice in gonococcal infections.

While the prophylaxis includes the use of contaceptives during sexual contact and maintaining hygiene of the genital areas such as always wash the genitals properly after sex, wear clean undergarments etc.

Gonorrhea can be correlated to “puyemeha” in Ayurveda. It is also known as “aupsargika meha”.

According to the Ayurvedic principles, puyemeha is an infective disease which spreads to the healthy person through sexual contact with an infected one. It occurs due to infection leading to hindrance of the natural balance of pitta levels which further disturb the natural balance of Rakta dhatu.

Its incubation period i.e. when the infection or bacteria has entered the body but has yet not produced any symptoms is considered to be 7 days.

It is considered that after the jeevanu (pathogen) enters the body it travels to the mutra marg resulting in shotha (swelling) which leads to vrana utpati (wound formation). This wound further presents puye srav (pus draining). This condition is known as puyemeha.

  • Sheeshan prakshalan(urethral wash) with herbal decoctions such as nimba patra kwath, phitkari jal etc helps in curing the condition.
  • There are various herbal remedies that are beneficial in puyemeha such as shweta parpati, puyemehari churna, sfateekadi churna etc.
  • Restrict sexual activities.
  • Ekantvas (Isolation) is suggested.
