What is Evans Syndrome?

Evans syndrome is a rare auto-immune disorder in which our own immune system starts forming antibodies against the healthy RBCs and platelets even destroy some WBCs.The destruction of cells is often called as Cytopenia. Affected people often suffer from a group of diseases like – Thrombocytopenia (too few platelets) and Coombs’ positive hemolytic anemia (premature destruction of RBCs), hence the name Evans syndrome.

Clinical Features of Evan Syndrome

Signs and symptoms may vary from person to person and also may depend upon the type of cells affected (Either RBCs, Platelets and/or WBCs) and the patient may present with the following signs and symptoms-

– Petechiae (tiny red dots) can appear at any part of the body.

– Bruises (blue spots), may be localized or generalized.

– Bleeding (can occur from any part of the body like – nose, oral cavity, hair follicles, eyes, etc.).

– The pallor of the skin

– Headache

– Vertigo

– Recurrent infections

– Shortness of breath

– Weakness

– Fatigue

– Rapid heartbeat

– Emotional distress

– Weight loss

Ayurvedic Perspective of Evans Syndrome

Evans syndrome is a Raktapitta disorder which means the responsible factors of this disorder are Rakta dosha (as per Sushruta) and Pitta dosha. Basically, Pitta acts as a vitiating factor that later leads to vitiation of the other two doshas in our body which are in Saptadhatus (especially the Rasa and Rakta dhatu).

Causes of Evans Syndrome

Evans syndrome is an auto-immune disorder and its exact cause is yet to be discovered but some factors may be responsible, which affect our digestive fire (Agni), the essence of all the dhatus (Ojas) and immunity, leading to the development of this disease in our body.

These factors can be –

Hereditary Factors (Vanshaj Karana) :

This disease can be due to heredity.

Environmental Factors :

Excessive exposure to UV rays, hot climate, pollution, etc.

Mental Status (Mansik Karana) :

Anger (Krodha), Grief (shoka), etc. leads to Pitta imbalance.

Lifestyle Factors (Vihaar Karana) :

Excessive exposure to sunlight (indulging in atapasevana), excessive walking (adhwa), excessive physical activities (vyayam), etc.

Dietary Factors (Aahar Karana) :

 Excessive consumption of oily, spicy, sour and fermented food like – Junk foods, bakery products, etc.

Diagnosis of Evans Syndrome

It is a diagnosis of exclusion which means that a diagnosis is made with Coombs-positive hemolytic anemia and thrombocytopenia in association with an abnormal immune response. Some tests can be done to differentiate it with other auto-immune disorders, having similar signs and symptoms –

– Blood tests (CBC, LFT, RFT, Platelet count, APTT, INR, Fibrinogen)

– Urine routine examination

– Antibodies assay

– Computed tomography

– Bone marrow Aspiration

– Biopsy

Evans syndrome can occur due to dietary factors, so some modification in the diet may decrease its severity and the second option can be some home remedies –

1.  Dietary Modification :

Take half teaspoon of each, saunf (Fennel), dhania (Coriander) and jeera (Cumin) or Anaar chilka (Pomegranate peel) in 3 cups of water and boil it till it is reduced to 1 cup. Now strain it and drink it. This decoction can be taken twice daily.
