Abstract

Liver is the main visceral organ of the digestive system. It is present in the right side of the abdominal cavity. It’s the largest organ of the digestive system. Likewise its size it performs many major functions in the body near about 500 vital functions are performed by the liver. One of the major functions is the production of bile and when the flow of bile from the liver is blocked to the other organs is known as cholestasis in this article we are going to know about the cholestasis, ayurvedic importance of liver and its treatment.

Introduction

Cholestasis is a disorder in which bile flow is obstructed from the liver and to the duodenum and other organs. Bile is a digestive juice which is produced by the liver transported to the duodenum and stored in the gallbladder through a common bile duct. Bile is very essential as it helps in the digestion of fats; absorption of different substances, some substances get secreted with the help of bile. So if the flow of this bile juice gets blocked these functions will be hampered in the body.

In Ayurveda

liver is named a yakrit. In ayurveda also it is a very essential organ. It is connected to the raktvaha strotas (channels of blood) of the body which means it is made from the rakta dhatu (blood). It’s the main organ for the chayaphchaya (metabolism). Any obstruction in the rakta vaha srotas can lead to the obstruction of the bile.

In ayurveda there is a disease which is known as kamala only this disease is mentioned in ayurvedic text. There are two types of kamala (harit samhita kc)

  • Koshtashrit kamla  (non-obstructive jaundice)
  • Shakha ashrit kamla (obstructive jaundice)

This cholestasis is a condition of obstructive jaundice and all the symptoms of obstructive jaundice are there in shakha ashrit kamla.

Types of Cholestasis

There are 2 types of cholestasis:

  • Intrahepatic cholestasis: It can occur due to any previous disease, infection or any use of drug which is hepatotoxic, genetic causes, bile flow etc.
  • Extrahepatic cholestasis: It can occur due to mechanical blockage of the bile duct. Blockage can be due to the cyst, gall stone or tumor.

Symptoms of Cholestasis

Common symptoms which are seen in the patients of cholestasis are:

  • Pruritus –itching of the body
  • Scratch marks on the skin due to itching
  • Jaundice
  • Fatigue
  • Xanthomas
  • White coloured stool
  • Nausea
  • Pain in abdomen

Cholestasis Causes

Probable causes are listed below

  • Hepatotoxic drugs
  • Hepatis
  • Pregnancy related issues
  • Taking  birth control pills
  • Usage of antibiotics
  • Gallbladder Stones
  • Cyst in the bile duct
  • Tumor in the bile duct
  • Congenital anomalies of the common bile duct.

Cholestasis Diagnosis

Diagnosis can be made on the basis of following tests:

  • Blood tests
  • Liver function test
  • Liver biopsy
  • Ultrasonography

Cholestasis Treatment

Treatment is based on different causes. Three factors for the treatments are

  • For any blockage of bile duct surgery is preferred.
  • For blockage which is intrahepatic certain medications can be used.
  • For the pruritis drugs like cholestyramine can be used.

Treatment in Ayurveda

There is no hepatoprotective drug in modern treatment but in ayurveda many herbs are mentioned which can be used for the treatment of liver diseases. Herbs like guduchi (Tinospora cordifolia), nimba (Azadirachta indica), gomutra (cow urine), daruharidra (Berberis aristata) and light virechana therapy (purgation) can be used for the treatment of the liver disorders. Trikatu can be used for the clearance of the channels in the liver. Many other treatments can be used like this which are mentioned in ayurvedic texts.

It is made using katuki (Picrorhiza kurooa), punarnava (Boerhavia diffusa), bhumi amla (Phyllanthus niruri) these herbs have evidence that these treat liver disorders and detoxifies it hence it will help in remove the strotoavrodha (remove obstruction from channels).

Dose – 1 tsp twice daily with warm water.

3. Livo Plan Syrup

This syrup is made using kalmegh (Andrographis paniculata), bhringraj (Eclipta Alba), bhumi amla (Phyllanthus niruri) etc. all these herbs are hepatoprotective herbs which have been proved to detoxify liver.

Dose – 2 tsp twice daily.

4. Arogyavardhini Vati

This medicine is mainly used for the detoxification of the blood. Its main component is katuki (picrorhiza kurroa) which is known as liver tonic.

Dose– 2 tablets twice daily.

5. Ajmodadi Churna

It is made using 12 herbs and main ingredients are ajmoda (Apium graveolens), Haridra (Curcuma longa), sonth (Zingiber officinale), pippali (Piper longum) etc. it will have effect on the inflammation inside the liver due to infections and will help in the reduction of the blockage.

Dose – 1 teaspoon twice daily.

6. Amrit Dhara

It’s an ancient preparation which is used for many disorders. For digestive related purposes you can use it with the combination of sugar or batasha with mixing 2 drops of the amritdhara.

Dose – 2 drops twice daily.

Cholestasis we saw is a disorder related to the stasis of the bile obstruction. It can produce many symptoms including jaundice. The main symptom is itching. In ayurveda we can see It can be related to the shakha ashrit kamla which is also a disease related to the obstruction of bile.
