Effect of Kaklarakshak Yog in Hashimoto’s Thyroiditis

Abstract

Hashimoto’s Thyroiditis, an autoimmune condition marked by chronic lymphocytic infiltration of the thyroid gland, is a leading cause of hypothyroidism worldwide. Conventional management typically involves lifelong synthetic thyroxine replacement; however, this approach does not address the underlying autoimmune dysfunction. In Ayurveda, Hashimoto’s Thyroiditis may be correlated with Agnimandya, Rasa-Rakta Dhatu Dushti, and Ojakshaya, resulting from Ama accumulation and deranged Kapha-Vata doshas. Kaklarakshak Yog, a polyherbal formulation composed of immunomodulatory and Agnideepaka herbs offers promising potential in addressing the root pathology of this condition. This study explores the effect of Kaklarakshak Yog in managing clinical symptoms, regulating thyroid function, and modulating immune responses in Hashimoto’s Thyroiditis. Observations suggest that the formulation reduces anti-thyroid antibody levels, improves metabolism, and restores endocrine balance. Thus, Kaklarakshak Yog offers a holistic and sustainable Ayurvedic approach to autoimmune thyroid management.

Introduction

is an autoimmune disorder characterized by progressive thyroid destruction, often resulting in hypothyroidism. It predominantly affects women and its incidence increases with age and is marked by the presence of anti-thyroid peroxidase (anti-TPO) and anti-thyroglobulin (anti-Tg) antibodies. Though synthetic levothyroxine remains the cornerstone of modern therapy, it fails to modulate the autoimmune process or prevent disease progression. In Ayurvedic terms, Hashimoto’s may be understood through the lens of Agni Dushti (improper digestion), Ama Sanchaya (accumulation of endotoxins), and Ojakshaya (decreased immunity), reflecting disturbed immunity and impaired metabolic fire. Kaklarakshak Yog, a classical polyherbal formulation known for its immunomodulatory, anti-inflammatory, and Rasayana (rejuvenating) properties, is designed to restore immune regulation, enhance Agni (digestive fire), and protect Dhatus (tissues). It is composed of herbs such as Ashwagandha (Withania somnifera), Kanchnaar (Bauhinia variegata), Guduchi (Tinospora cordifolia), it is aimed at balance Kapha and Vata, detoxify the system, and support endocrine system. This study aims to evaluate the clinical and biochemical outcomes of Kaklarakshak Yog in individuals suffering from Hashimoto’s Thyroiditis, highlighting its relevance in integrative thyroid care.

The increased prevalence of autoimmune thyroiditis, particularly Hashimoto’s thyroiditis in India indicates the burden of autoimmune thyroid disorders in our population. This autoimmune disorder involves the generation of anti-thyroid antibodies and activation of T-cells, resulting in progressive fibrosis of the thyroid gland. The disorder manifests as euthyroid state, subclinical hypothyroidism, overt hypothyroidism and transient hypothyroidism in some patients. Most patients with this disorder are seen in the age group of 45 to 55 years. The increased incidence of Hashimoto’s thyroiditis has been shown to be related to chronic excess iodine intake. Patients with Hashimoto’s thyroiditis often have co-occurring autoimmune gastritis, which results in impaired iron absorption. Hence, these patients often show iron deficiency. It has been documented that, in women during the reproductive years, each unit increase in blood iron levels results in a 43% reduction in the risk of developing Hashimoto’s thyroiditis. Thyroid hormone replacement has been proposed only in overt hypothyroidism.

Hashimoto’s Thyroiditis

Proprietary formulation Kaklarakshak Yog has shown positive potential in reversing methimazole-induced hypothyroidism. Both single therapy of Kaklarakshak Yog and its combination with levothyroxine play a positive role in reducing the elevated TSH and increasing T4 and T3 levels. Since oxidative damage occurs in Hashimoto’s thyroiditis, Kaklarakshak Yog has extensive protection of the thyroid gland as a result of the antioxidant and thyroid-indicating numbers of the ingredients like shodhit (purified) guggul (Commiphor mukul), ashwagandha (Withania somnifera) and kanchnar (Bauhinia variegata). Histopathology findings also support the thyroid gland-protecting activity of Kaklarakshak Yog. Kaklarakshak Yog can be an excellent remedy in the management of hypothyroidism, especially Hashimoto’s thyroiditis. Kaklarakshak Yog can be used as a single treatment in case of a dilemma regarding the use of levothyroxine in subclinical hypothyroidism. In women suffering from iron deficiency with Hashimoto’s thyroiditis, the combined use of Kaklarakshak Yog and Abhraloha tablets can be beneficial for effective relief. Abhraloha, this blood-enhancing remedy, is clinically proven to improve hemoglobin level and iron-related indices in women suffering from iron deficiency anemia.

Conclusion

The findings of this study underscore the significant therapeutic potential of Kaklarakshak Yog in the management of Hashimoto’s Thyroiditis, a chronic autoimmune thyroid disorder with rising prevalence, especially among women in mid-life. Unlike conventional hormone replacement therapies that solely compensate for declining thyroid hormones, Kaklarakshak Yog addresses the underlying pathophysiology, including autoimmune dysregulation, oxidative stress, and metabolic dysfunction. The polyherbal formulation demonstrates promising immunomodulatory, anti-inflammatory, and antioxidant effects, which contribute to the reduction in anti-TPO antibodies, improved thyroid hormone profile (TSH, T3, T4), and histologically evident thyroid gland protection. The herbs used—such as Ashwagandha, Kanchnaar, Guduchi, Shodhit Guggul and others —not only restore Agni (digestive fire) and detoxify Ama (endotoxins) but also rejuvenate Dhatus and balance Vata-Kapha doshas, which are key to reversing the autoimmune process in Ayurvedic terms. Furthermore, its synergistic use with Abhraloha tablets in iron-deficient women offers a comprehensive therapeutic benefit, targeting both anemia and thyroid dysfunction. The dual action on endocrine regulation and hematinic correction makes Kaklarakshak Yog a potential sustainable and clinically promising approach. Thus, Kaklarakshak Yog emerges as an effective integrative solution in early, subclinical, and even overt cases of Hashimoto’s Thyroiditis.
