Two types of NAFLD ( nonalcoholic fatty liver), or NAFL (also referred to as metabolic dysfunction-associated steatotic liver and nonalcoholic steatohepatitis, or NASH (also referred to as metabolic dysfunction-associated steatohepatitis . People typically develop one type of NAFLD.
1. NON ALCOHOLIC FATTY LIVER
NAFL is a form of NAFLD in which you have fat in your liver but little or no inflammation or liver damage because it not happening due to alcohol intake. NAFL does not progress to cause liver damage or other complications. NAFL can cause pain from enlargement of the liver.
2. NONALCOHOLIC STETOHEPATITIS
NASH is the advanced form of NAFLD in which you have inflammation of the liver and liver damage, in addition to fat in your liver. The inflammation and liver damage of NASH can cause fibrosis of the liver.
Stages of Fatty Liver Disease
- Simple Fatty liver or steatosis :- This is the first stage of fatty liver. There are often no symptoms in early stage symptoms, with a healthy diet and regular exercise, excess fat in liver cells can be reduced.
- Second stage of fatty liver- when fat build up in liver cells is accompanied with inflammation. Inflammation occurs when the liver is repairing damaged cells. If the amount of damaged tissue increases, the liver may struggle to repair it fast enough so inflamed tissue remains scarred.
- Third stage of fatty liver :- when there is persistent scar tissue in liver and in blood vessels around the liver. Liver still functions quite well at stage,removing or treating the cause of inflammation may prevent further progression or even some damage. Function of the liver is affected.
- Fourth stage of liver:- At this stage , liver stops working properly, systems start to appear such as yellowing of the skin and whites of the eyes as well as dull ache in lower side of ribs at this stage there is more risk to develop cirrhosis.
Causes
- Many kinds of genetic and environmental factors [diet + sedentary lifestyle] play a role in progressing the disease.
- Diet :- Excessive consumption of fatty and fried food ,bread, rice, potatoes, sugar rich drinks such as cold drinks and alcohol containing drinks. Excess use of dairy products and heavy food like non veg etc.
- Drugs :- long term usage of drugs like steroid, immunosuppressive and certain chemotherapy drugs could cause liver damage.
- Some surgical procedures can cause fatty deposition in the liver.
- HELLP syndrome:- It is a Rare Pregnancy complication that mainly affects your blood and liver. Three signs of this disease (Hemolysis, Elevated liver enzyme, Low Platelets count).
- Others:- Congenital and Inherited liver diseases etc.
Symptoms
NAFLD is frequently asymptomatic, When it does, they may include:-
- Fatigue and sometimes pain in mild right upper quadrant discomfort
- Loss of appetite
- Nausea due to indigestion
- Vomiting
- Confusion
- Poor judgment
- Trouble in concentrating
Diagnosis of Fatty liver Disease
- Blood investigation:- If blood tests show a high level of AST and ALT. Serum GGT is also raised.
- Ultrasound imaging:- Ultrasound is most often used and provides a qualitative assessment of hepatic fat content.
- Computed tomography and Magnetic resonance imaging. These kinds of imaging create the image of the liver and give grading of fatty liver. They offer great sensitivity for lesser degree of steatosis ( fat accumulation)
- Fibroscan:- FibroScan is a non-invasive device that assesses the ‘hardness’ (or stiffness) of the liver via the technique of transient elastography.
- Liver biopsy:- Although it is not frequently required for either diagnosis and staging. But we can Assess the degree of inflammation and extent of liver fibrosis.
Treatment
Treatment for NAFLD usually starts with weight loss. This can be done by eating a healthy diet, limiting portion sizes and exercise. Losing weight may improve other health problems that lead to NAFLD. By avoiding medicine that can affect our liver. Along with this, quit smoking and drinking alcohol.
- Lose weight :- By reducing the number of calories each day and increase the physical activity to lose weight slowly. Eating fewer calories is key to losing weight and managing this disease.
- Exercise :- Aim for at least 30 minute walk everyday. If will increase metabolism of body that will be helpful in reducing weight .

Ayurvedic overview of fatty liver disease
According to Ayurveda Yakrit (liver) is the place of origin or centre of production which maintains the Pitta dosha in our body. When the Rasa dhatu (first tissue formed in gut in form of essence of food due to action of agni) enters the yakrit and pleeha (liver and spleen respectively), further action by Ranjaka Pitta it give Red colour to it and convert into rakta dhatu . Thus Yakrit (Liver) is responsible for the maturation of Rakta. Yakrit (Liver) controls many activities in the body.The basic temperature of Yakrit is hot owing to the presence of Pitta. But when its temperature disturbed due to our Ahara vihar (diet + environment) then coldness prevails and dominates the hot zone of Yakrit. Further it slows or sluggish its activity. Therefore, it can be inferred from this correlation that fatty liver disease results from elevated Kapha dosha. This gives rise to many diseases related to Yakrit.
Causes
- Kapha Prakopaka causes ( Factor those increase level of kapha )
- Divaswapna :- Sleeping during daytime
- Alasya:- Excessive laziness
- Avayama:- No activity especially after eating heavy food Consumption of these food influence the progression of fatty liver disease
- Madhura Ahara :- Sweet taste, especially cold drinks.
- Snigdha Ahara :- Oily or fatty food ( junk food )
- Guru Ahara :- Heavy to digest ( non veg )
- Abhisyandhi Ahara :- Which blocks the body’s channels ( Curd)
Thus we can say Kapha increasing food and activity gives progression of Fatty liver disease or avoiding or minimizing the quantity of these foods can help in reducing the cases of FLD.
Sign and symptoms
- Ajirna :- Which is known as indigestion.
- Stauthlya :- Presentation seen in obese people.
- Amlapitta:-Fatty liver progression leads to Amlapitta (Hyperacidity)
