CAUSES

Metastatic Crohn’s disease is thought to be caused by extraintestinal spread of Crohn’s fistulas, as well as by the development of circulating cells and antibody-producing cells within the body. More recently, it has been recognized that tumor infiltrating lymphocytes (TIL) are also produced, and that TILs may play a role in the initiation of the disease. Metastatic Crohn’s disease tends to affect both men and women equally. The average age at diagnosis in most of the caes is 70 years, however, the most common age observed at onset is 60 years. This will lead to the presentation of a variety of symptoms described as follows.

SYMPTOMS

The clinical presentation of the condition of Metastatic Crohn’s disease can be different from patient to patient. The most common presentation is chronic disease of the skin (particularly the hands and feet), followed by the gastrointestinal tract (often presenting as abdominal pain). Other extraintestinal manifestations of Crohn’s disease include respiratory problems such as asthma. Another rare presentation of Crohn’s Disease is called Proctitis fibrosa cystica. In this clinical presentation, the patient presents with a primary rectal mass and a co-morbid proliferative disease elsewhere in the body. This form of Crohn’s Disease is very challenging to diagnose due to its rarity. There may be a variety of presenting features depending upon the site involved, it is important to diagnose it properly and manage accordingly.

DIAGNOSIS AND TREATMENT

Initially antibiotics are administered but in severe cases, unmanageable by medicines, surgical intervention may also be required.

AYURVEDIC ASPECT

In the ayurvedic aspect, we can correlate the condition of metastatic crohn’s disease with the vitiation of the pitta dosha which further vitiates the other dosha and different components of the body. This will result in various difficulties such as weakness, weight loss, burning sensation, etc.  The bhrajaka pitta is believed to be residing in the skin and the skin manifestations of this disease can be a vitiation of the bhrajaka. This vitiation of pitta dosha hinders the state of the body as it is responsible for digestion, and various other essential bodily features. To manage the condition of metastatic crohn’s disease, it is necessary to rebalance the equilibrium between the vitiated dosha of the body. This aim can be achieved through the administration of ayurvedic formulations according to the type of the body.
