How Can Phimosis Be Managed In Ayurveda With Herbal Remedies?
What Is Phimosis?
Phimosis is the painful condition characterised by inability of the person to retract the foreskin of the penis. There is a chance of injury to the penis when this adhered foreskin is retracted forcefully. Phimosis can also occur physiologically in infants. But this condition may get corrected on its own with advancement of the age i.e. resolved at the age seven or may by age of 16.
This disease has a very good correlation in Ayurvedic texts with Nirudhpraksh. Acharyas had described the causes, pathophysiology and clinical features along with treatment of nirudhprakash in brief, which will be discussed later on in this article.
Sign and Symptoms Of Phimosis
Clinical features of phimosis can vary from person to person. Few of the common manifestations include swelling of the foreskin, difficulty in urination or semen ejaculation, inflammation at the tip of penis, pain while erection, inability to pull the foreskin back, bruising may appear at the tip of penis, and high grade fever (> 101 ° F).
Causes Of Phimosis
The exact cause of this disease is not known, it most commonly affects the younger individuals during old age. Few factors considered responsible for the development of phimosis are aging– with increase in age there is a gradual loss of elasticity of skin leading to the phimosis in the persons with very less erections, recurrent pulling and stretching the foreskin can cause tiny tears on foreskin which later turns into inflammation and ended up as a phimosis. Several diseased conditions such as diabetes or balanitis (penis tip infection) can also lead to phimosis. Also several infections (e.g. sexually transmitted diseases) of the penis can result in scar tissue formation which makes the foreskin skin less stretchy and harder, which also is a factor contributing to the phimosis formation.
Ayurvedic View On Phimosis
As mentioned earlier, Phimosis is best correlated with the condition Nirudhpraksh mentioned in many Ayurvedic texts. It is also termed as nirudhmani. Acharya Sushruta had given the brief detail about the diseases as Due to atimardan– excessive rubbing, peedan -pressing or abhighatadi (traumatic etc.) causes lead to the vitiation of vata dosha which got localise to glans (mani), and prepuce (shishangra twacha), and produce adhesions (aashlesh) in both of them, resulting in blockage/obstruction in the passage of urine (mutrastrotso rundhi), which is characterised by thin stream of urine without excessive pain (manddhaaramavednam) and the glans can’t be exposed. Then this diseased condition is termed as Nirudhpraksh. This disease is also believed to be caused due to the incomplete recovery from the injuries of a disease named Avapatika (paraphimosis – a condition opposite to phimosis where the foreskin is retracted back from the glans or tip of penis and remains stuck there).
Whenever, the kid or the person affected from this disease try to retract the foreskin forcefully, it may result in breaking down of those adhesions (scar tissue) and which leads to severe pain, commonly followed by bruising and injuries to the glans and the prepuce. What is Balanitis?
