Abstract

Introduction

Segmental Colitis

SCAD is mostly seen in people of old age. Due to streamlined culture, asymptomatic SCAD occurs in over 50% of the population above the age of 70 years. It is rare under 30 years of age and is very common in western societies. SCAD is often asymptomatic and may be detected as an incidental finding. However, the vast majority of the population develops symptoms.

Pathophysiology

A life-long refined diet with a low quantity of fiber is widely thought to be responsible and the condition is rare in the very high fibrous intake.

Based on the etiologic role of a low-fiber diet, the pathogenesis of SCAD is-

  • Due to the low fiber content of the diet, there is excessive intra-colonic pressure to pass the low volume stool leading to excessive segmentation and thereby sequestration of mucosa and submucosa.
  • The muscular weakness of the colonic wall at the junction of submucosa with muscular.
  • Due to less collateral blood supply in the colon, SCAD occurs commonly in the small intestine than in the large intestine.
  • Diverticula consist of protrusions of mucosa covered with peritoneum thus, the circular muscle coat gets hypertrophied.
  • Due to the impaction of diverticula with fecoliths, inflammation occurs.

Clinical Features

The symptoms usually occur due to associated constipation or spasm.

  • The foremost symptom is rectal bleeding. The bleeding may be profuse. One must keep in mind that profuse rectal bleeding in the elderly is more often due to SCAD than carcinoma.
  • As the disease progresses, due to repeated attacks of inflammation, leading to thickening of the bowel wall, narrowing of the lumen, and eventually, obstruction; the presentation varies.
  • Intermittent abdominal pain, usually colicky in the suprapubic or left the iliac region with flatulence and slight distension is the characteristic feature. The pain can be due to overdistension or localized abscess.
  • The sigmoid colon may be palpable and during pain attack, there may be tenderness, guarding, and rigidity, and sometimes a palpable mass is known as left-sided appendicitis.
  • During the episode of pain, there may be diarrhea or profuse rectal bleeding.
  • In some, the bowel actions are small and leave a feeling of incompletion. The patient passes small, pebbly motion which may be accompanied by blood and/or mucus.

Complications

Any disease if not treated at the time and left for growing several complications does develop and make the disease more difficult to be treated. Similar is the case here:

  • Fibrosis & adhesion along with a colonic abscess
  • Intestinal obstruction due to a pelvic mass which may be palpable both per abdomen and per rectum
  • Necrosis of diverticulum which ultimately leads to perforation
  • With the perforation, peritonitis may develop
  • Vesico-colic or entero-colic fistula along with peritonitis

The above mentioned are more common in people who take NSAID’s or aspirin

Investigation

  • Barium meal follow-through or barium enema may be helpful. In the early stage, there may be small saw-tooth projections. In the later stage, large sawtooth will be obvious.
  • CT scans may be done to diagnose any complication.
